The Access Services online forms, like the Library Resources online forms, are not currently working correctly.
1. For all Associate Card requests: fill out the manual form at the Service Zones and leave in the tray on the SZ3 shelving.
2. For all other Access Services online forms eg Alumni, fill out the form (either the client if present or over the phone), print the form and put the printout in the Associate Card requests tray on the shelving in SZ3.
3. If at all possible to determine, note on the printout what failed to work eg: an Alumni request I filled out recently allowed me to fill all the fields except the Surname field. Record this information on the printout including the date and time.
4. We will be advised when the problem with online forms is resolved.
